Support Provider Match-Up Form

The Support Provider Match-Up Form is now available for download! The site administrator must complete the form and return it to the Teacher Support Unit for verification. Please click here to download the match-up form.

 

 

SUPPORT PROVIDER VERIFICATION OF DUTIES FORM

In order to receive the stipend for work as a Support Provider, the school site administrator must submit a verification of duties form listing each Support Provider eligible to be paid the stipend. The completed form should be returned to the Teacher Support Unit by January 16, 2009.

In order to support you in your work, the LAUSD BTSA Induction Program provides professional development for Support Providers.  The three major components of your support provider training are:
  • Support Provider Orientation*
  • CFASST Training and/or FACT Training
  • Culturally Relevant & Responsive Pedagogy

* Support providers are required to attend at least one Support Provider Orientation.
